What this sheet is for

When one person carries most of the caregiving load, small gaps in coverage — a medical appointment, a few hours of sleep, an errand — can become a serious problem. This sheet lets family members, friends, neighbours and community volunteers put their name down to provide respite cover, giving the coordinator a clear picture of who has offered help and how many people are available for the period ahead.

How to use it

Fill in the Month / Week field at the top right before circulating the sheet. Ask anyone willing to provide respite cover to print their name on a numbered line under Volunteers 1–15 or Volunteers 16–30 — one person per line. The note line above the columns reminds signatories that a coordinator will follow up to assign specific dates and times, so no one is committed to a slot they cannot fill. Count all names at the end and record the figure on the Total volunteers listed line at the foot so you know your pool size at a glance.

What is printed on the page

“Respite Care Volunteer Sign-Up” is laid out as multi-column: two side-by-side columns of fifteen numbered name lines — thirty names on one page with no contact column — a fill-in field at the top right and a total line at the foot.
