What this sheet is for

When you are the primary carer for a parent or patient, appointments arrive from several directions at once — specialist clinics, physiotherapy, district nurses, social workers, and delivery services for equipment or prescriptions. A single monthly view lets you spot clashes before they happen, confirm that recurring visits are not falling through the gaps, and hand the page to a relief carer without a lengthy verbal briefing.

How to use it

Write each appointment in the matching date cell: include the provider's name, time, and location or whether it is a phone or video call. Use the Upcoming Appointments box at the foot to list dates confirmed but not yet scheduled with a time, so nothing is forgotten when you build next month's sheet. Record the GP, specialist, district nurse, and any other regular contacts in Care-Team Contacts so anyone covering for you can reach the right person quickly. Use Supply and Cost Notes to log equipment orders, delivery dates, out-of-pocket expenses, and insurance claim reference numbers for the month.

What is printed on the page

“Monthly Care Appointments” is laid out as classic month grid: a full-width 7 × 6 month grid with a date corner in every cell and three labelled boxes across the foot (e.g. key dates, goals, notes). Under the title it reads “Track all medical visits, therapy sessions, and home-care calls for one month.”, and the line at the foot of the page says “After each appointment, note any follow-up instructions or new referrals in that day's cell before you forget.”.

The box headings slot holds three entries — “Upcoming Appointments”, “Care-Team Contacts” and “Supply & Cost Notes” — so the sheet is already filled in for caregiving rather than left as blank rows to label yourself.

The remaining labels on “Monthly Care Appointments”, the caregiving sheet, are “Month / Year” for the month label.

Two things about this particular classic month grid are settings rather than text, and “Monthly Care Appointments” has them set like this. Start sunday is off: start the week on Sunday instead of Monday. Bottom boxes is on: include the three boxes under the grid (off gives the grid the whole page).

Tips

Write transport arrangements beside each appointment — who is driving or which service to book — so a relief carer has everything in one place.

Circle any appointment where you need to bring a written symptom summary, recent test results, or a list of current concerns for the clinician.

At month end, total costs noted in Supply and Cost Notes and transfer the figure to your annual care-expense log before filing this sheet.
