What this sheet is for

When you are managing care for a parent or patient, appointments multiply fast — GP reviews, specialist consultations, physiotherapy, district nurse visits, blood draws, and more. Missing a follow-up or forgetting what was decided at the last cardiology visit has real consequences. This log keeps every encounter in one place so nothing is lost between appointments or between carers.

How to use it

Write the patient name and primary care facility in the target fields at the top. In the Date column, enter the appointment date. Under Appt. Time, note the scheduled time and the name of the clinician or department seen. Use Outcome / Next Step to record the key finding, decision, or instruction given, and any action required — a referral sent, a test ordered, a medication review booked, or a return date given. Work through both columns left to right; the sheet holds 36 entries, roughly three to six months of active carer use.

What is printed on the page

“Caregiver Appointment Log” is laid out as two-column log: two side-by-side tables of date, value and note (18 rows each, 36 entries a page), with a target field across the top. Under the title it reads “Record every medical visit, outcome, and follow-up for one patient.”, and the line at the foot of the page says “Keep the most recent three months of logs in the care folder so any substitute carer or new clinician can review them quickly.”.

The remaining labels on “Caregiver Appointment Log”, the caregiving sheet, are “Month / Year” for the period label, “Appt. Time” for the value heading, “Outcome / Next Step” for the note heading, “Patient name” for the target prompt and “Care facility / GP” for the unit prompt.

Two things about this particular two-column log are settings rather than text, and “Caregiver Appointment Log” has them set like this. Target row is on: include the target and unit fields across the top. Weekly dividers is off (the opposite of the layout's default): draw a heavier rule every seven rows.

Tips

Write the follow-up deadline in the Outcome column and circle it so urgent actions stand out when you scan the page.

Record who attended each appointment — a second family member or advocate — in case their account of advice differs from yours later.

If an appointment is cancelled or rescheduled, draw a single line through that row and enter the new date immediately below it.
