What this sheet is for

When you are caring for an elderly parent or a patient at home, the details of each day — what was eaten, how they slept, a new skin complaint, a phone call to the district nurse — blur quickly into one another. This sheet gives you a single page to anchor each calendar day to its care events, so nothing is lost when a professional asks what changed and when.

How to use it

Write the month and year at the top right. Use each day cell in the main grid to note care tasks completed, any symptom or behaviour changes, and supplies that ran low. Keep the four lines under This Month's Care Goals for outcomes you are working toward with the care team — improved mobility, better sleep, wound healing. Tick off the six items in the Monthly Admin Checklist as you handle them across the month. Use Next Month to Prepare to flag anything that needs booking or ordering before the page turns.

What is printed on the page

“Monthly Caregiver Task Log” is laid out as grid with sidebar: a 7 × 6 month grid taking two-thirds of the width, with a sidebar of four goal lines, a six-item checklist and a reminder box. Under the title it reads “Daily care tasks, symptoms, supply needs and care-team contacts for one month.”, and the line at the foot of the page says “File this sheet with the month's receipts and any discharge or clinic notes before storing it.”.

Two things about this particular grid with sidebar are settings rather than text, and “Monthly Caregiver Task Log” has them set like this. Start sunday is off: start the week on Sunday instead of Monday. Reminder box is on: include the reminder box at the foot of the sidebar.

Tips

Use a single lowercase letter code — s for sleep issue, a for appetite, f for fall — to keep day cells readable.

Write supply reorder notes in the day cell the moment the low level is noticed, not at month end.

If two people share caregiving duties, each should initial their entries so the record stays attributable.
