What this sheet is for
When you are caring for an elderly parent or a patient at home, spending accumulates in small, easy-to-forget transactions: a prescription co-pay here, an incontinence supply run there, a home-health aide invoice on a random Tuesday. This sheet gives every day of the month its own line so that nothing slips through before you reconcile costs, file for reimbursement, or prepare records for a benefits review.
How to use it
At the top, write the month and year, then note the main expense category you want to watch closely in the 'Main expense to watch this month' box — for example, transport costs or agency fees. Each day has a numbered line: write the item purchased or service paid, the provider or store name, and the amount in your local currency. The heavier rules every seven days let you subtotal by week if needed. At month end, answer the two prompts under 'End-of-Month Review' to identify budget pressure points and any costs eligible for reimbursement or tax relief.
What is printed on the page
“Monthly Care Costs Log” is laid out as day list: days 1–31 as two columns of numbered writing lines, a 'focus this month' box at the top and a two-prompt review at the bottom. Under the title it reads “Track out-of-pocket caregiving expenses, invoices, and supply purchases by day.”, and the line at the foot of the page says “Keep receipts in a dated envelope each month — many caregiving costs qualify for tax deductions or reimbursement claims.”.
The review prompts slot holds two entries — “What was the single largest unexpected cost this month?” and “Which recurring expense could be reduced or claimed back?” — so the sheet is already filled in for caregiving rather than left as blank rows to label yourself.
The remaining labels on “Monthly Care Costs Log”, the caregiving sheet, are “Month / Year” for the month label, “Main expense to watch this month” for the focus prompt, “Item · provider or store · amount paid” for the column hint and “End-of-Month Review” for the review heading.
Two things about this particular day list are settings rather than text, and “Monthly Care Costs Log” has them set like this. Review box is on: include the review box at the bottom. Weekly dividers is on (the opposite of the layout's default): draw a heavier rule every seven days.
Tips
Write amounts in a consistent currency and note any foreign or online purchases that carried a conversion fee.
If a single day has multiple expenses, use a slash to separate them on one line rather than leaving other days blank.
Circle any entry that needs a receipt scanned or a reimbursement form submitted so nothing is missed at month end.
