What this sheet is for

When you are coordinating care for an elderly parent or a patient at home, the list of involved people grows quickly: a GP, a specialist, a district nurse, a physiotherapist, a social worker, a pharmacy contact, and perhaps a neighbour who checks in. This sheet keeps every name, number, and availability note in one place so any family member or relief carer can find the right person fast.

How to use it

Write the date you last verified the details in the Last updated field — contacts change more often than expected. In Name & Role, be specific: write 'Dr A. Mensah — cardiologist' rather than just a name. Use Phone / Fax to record direct lines and fax numbers for referral forms. In Availability, note days and hours, or a note such as 'after-hours via service'. Assign a Status code from the legend: AC for anyone actively involved, SB for someone on standby, RE for a referral not yet followed up, and IN for contacts no longer part of the care. Count the AC entries for the Total active contacts tally at the foot.
