About these caregiving Habit Tracker sheets
These three sheets cover the daily and weekly record-keeping that home caregivers most often need but rarely have a ready format for. The Daily Elder Care Checklist gives you a month-long grid for confirming that core care tasks were done each day. The Weekly Caregiver Self-Check turns the lens on the caregiver, tracking sleep, meals, and coordination tasks. The Daily Hydration Check focuses on a single, frequently overlooked measure: whether your parent or patient met their fluid intake target.
Choosing between them
Start with the Daily Elder Care Checklist if you need one place to confirm that hygiene, meals, mobility, and safety checks happened across a full month — useful whether you provide care yourself or hand off to another family member or aide. Add the Daily Hydration Check if a doctor has flagged dehydration risk or you simply want a cleaner, dedicated record for fluid intake. Use the Weekly Caregiver Self-Check alongside either of the others if you are the primary caregiver and your own sleep, meals, or coordination tasks are starting to slip.
What each sheet contains
“Daily Elder Care Checklist” is the 30-day grid layout — ten labelled habit rows against 31 numbered day columns of tick boxes, an optional totals column, and a notes box at the foot. Its habit labels run to ten entries, starting with “Morning personal hygiene”, “Meal eaten — breakfast” and “Meal eaten — lunch”.
“Weekly Caregiver Self-Check” is the weekly grid layout — eight habit rows against seven day columns of large tick boxes, a streak count box, and a three-prompt reflection for the week. Its habit labels run to eight entries, starting with “Slept at least 6 hours”, “Ate a proper meal (not just snacks)” and “Left the house for 15+ min”.
“Daily Hydration Check” is the single habit layout — one habit written large at the top, 31 big numbered day boxes in rows of seven, a 'why this matters' box, a 'when I slip' box and three milestone lines. Its milestone labels run to three entries, starting with “7 days met”, “14 days met” and “30 days met”.

Each of the three caregiving Habit Tracker sheets downloads as a PDF in US Letter or A4 from its own page, laid out in black and white so it prints on any home or office printer. Nothing on the page is carried by colour, the fields are sized for handwriting, and the same layout can be refilled with your own labels in the generator.
