What this sheet is for

When you are caring for an older adult, fluid intake is easy to overlook on a busy day — yet inadequate hydration is one of the most common and preventable causes of hospital visits in this group. This sheet gives you one clear habit to anchor your routine: confirming the daily fluid target was met before you close out the day. It is simple enough to keep on a kitchen counter or beside the care recipient's chair.

How to use it

Write the agreed daily fluid target on the 'Daily fluid target' line at the top — for example '1.5 L / 50 oz' or '6 cups'. Cross or tick each numbered day box once you have confirmed the target was reached. In the 'Why consistent hydration matters for this person' box, note any specific reason the doctor has flagged — UTI history, kidney function, heat sensitivity. Use 'What I will do if I miss logging a day' to plan a fallback, such as asking a co-carer to confirm and mark the box. Record the date and a brief note when you hit each streak milestone.

What is printed on the page

“Daily Hydration Check” is laid out as single habit: one habit written large at the top, 31 big numbered day boxes in rows of seven, a 'why this matters' box, a 'when I slip' box and three milestone lines. Under the title it reads “Track whether your care recipient drank enough fluid each day this month.”, and the line at the foot of the page says “Dehydration in older adults can mimic confusion or fatigue. Eight 240 ml / 8 oz cups is a common daily target — confirm the right amount with their doctor.”.

The milestone labels slot holds three entries — “7 days met”, “14 days met” and “30 days met” — so the sheet is already filled in for caregiving rather than left as blank rows to label yourself.

Tips

Set a phone reminder for the same time each evening so reviewing fluid intake becomes a closing routine before the care day ends.

If the person resists plain water, note accepted alternatives — broth, herbal tea, fruit — in the 'why' box as a personal reference.

When handing over to a relief carer, show them the sheet so the log stays unbroken and they know the day's status at a glance.
