About these caregiving Checklist sheets

These three printable checklists cover the main pressure points in unpaid caregiving for an adult at home. The Daily Home Care Checklist structures every shift from morning routine to evening handover. The Weekly Care Team Prep sheet coordinates appointments, contacts, and supplies across a full week. The Hospital Discharge Transition checklist guides carers through a one-time but high-stakes move from ward to home. Each sheet works independently; together they cover day-to-day care, weekly planning, and a critical transition.

Choosing between them

Start with the Hospital Discharge Transition checklist if your parent is leaving hospital in the coming days — the phased timeline holds the most urgent information. Once care at home is established, print the Daily Home Care Checklist for anyone who provides hands-on support, whether that is you, a family member, or a paid carer covering a shift. Add the Weekly Care Team Prep sheet when more than one person is involved in coordinating care, or when the number of appointments and supply runs makes week-by-week planning necessary.

What each sheet contains

“Daily Home Care Checklist” is the single column layout — three headed sections of seven pre-filled checkbox items each down one column, optional blank extra lines per section, and a notes box at the foot. Its section 1 items run to seven entries, starting with “Checked overnight incontinence pad or sheet — changed if needed”, “Offered water or preferred morning drink within 30 min of waking” and “Noted and recorded morning blood pressure and pulse”.

“Weekly Care Team Prep” is the two columns layout — four headed sections of eight pre-filled checkbox items each, two sections per column, with a notes box across the foot. Its section 1 items run to eight entries, starting with “GP / primary care visit confirmed”, “Specialist appointment transport arranged” and “Physiotherapy session time noted”.

“Hospital Discharge Transition” is the phased timeline layout — four time-phased sections (e.g. '8 weeks before', 'Day of') of six pre-filled items each, with an optional done-date column on the right. Its phase 1 items run to six entries, starting with “Request a written discharge summary from the ward nurse”, “Confirm all new prescriptions and ask for a printed list” and “Ask the physiotherapist about mobility aids needed at home”.

Printing

Each of the three caregiving Checklist sheets downloads as a PDF in US Letter or A4 from its own page, laid out in black and white so it prints on any home or office printer. Nothing on the page is carried by colour, the fields are sized for handwriting, and the same layout can be refilled with your own labels in the generator.
