Baby's First-Year Milestone Log

Daily guided journal page for tracking a baby's milestones, feeding, sleep, physical condition, and development across the first year.

What this sheet is for

The weeks after a baby's birth move fast and blur together. This journal page gives parents and carers a structured place to record one day at a time — not just what happened, but what it meant. It is especially useful during the newborn period and at developmental leaps, when small changes feel significant but are easy to lose track of without a written record.

How to use it

Write the date and your baby's age in weeks or months at the top right. Circle one label on the 'Baby's overall day' scale to capture the day in a word before you write anything longer. Work through the five prompts in order: new behaviours, feeding detail, sleep pattern, physical observations, and your interaction with your baby. Use as much or as little of the ruled space as feels right. Finish by writing one concrete intention in the 'Tomorrow I want to try' box — a feeding adjustment, a new activity, or a question to raise with your health visitor or paediatrician.

What is printed on the page

“Baby's First-Year Milestone Log” is laid out as guided prompts: a five-point labelled scale at the top, five prompts each with ruled writing lines beneath, and a closing box at the foot. Under the title it reads “A daily reflection on your baby's development, mood, and new abilities.”, and the line at the foot of the page says “Keep one completed page per week in a folder — six months of entries will show patterns you would otherwise forget.”.

The scale labels slot holds five entries — “Unsettled”, “Fussy”, “Steady”, “Content” and “Thriving” — so the sheet is already filled in for baby rather than left as blank rows to label yourself.

The prompts slot holds five entries — “What new thing did my baby do or respond to today — a sound, movement, or expression?”, “How did feeding go? Note times, duration or volume, and any changes from yesterday.”, “Describe sleep in the last 24 hours: longest stretch, total naps, and how baby settled.”, “How is my baby's body today — skin, fontanelle, cord or circumcision site, bowel output?” and “What did I do to support my baby's development today, and how did baby respond to it?” — so the sheet is already filled in for baby rather than left as blank rows to label yourself.

The remaining labels on the page are “Date / Age” for the date label, “Baby's overall day” for the scale heading and “Tomorrow I want to try” for the closing prompt.

Two things about this particular guided prompts are settings rather than text, and “Baby's First-Year Milestone Log” has them set like this. Mood scale is on: include the five-point scale at the top. Closing box is on: include the closing box at the foot.

Tips

Fill in the scale and feeding prompt immediately after the last feed of the day, while the detail is still clear.

For the physical observations prompt, use specific terms: colour, texture, frequency, and any odour change in nappy output.

Photograph the completed page monthly — a searchable image is easier to share with a doctor than a folder of loose sheets.

The other layouts in this set

“Baby's First-Year Milestone Log” is one of three sheets in this set, each a different fixed layout filled for the same topic. “Baby Sleep & Feed Daily Log” uses the two-column reflection layout and “Newborn Care Daily Notes” uses the daily log journal layout. Pick by how you work: the layout decides how much you write and where, not what the sheet is about.
