What this sheet is for

In the early weeks and months, each day blurs into the next. This sheet gives a tired parent one focused page to record what actually happened — when the baby fed, how long each sleep lasted, how many nappy changes there were, and a moment to note how the caregiver is holding up. Filling it in takes two minutes and gives you something concrete to show a midwife, health visitor, or GP if any concerns arise.

How to use it

Write the date and your baby's age in weeks at the top right. Use the left column to answer the three short prompts — last night's sleep in plain words, any feeding concerns such as poor latch or unusually low intake, and an honest note on your own energy. Use the ruled right column headed 'Times, notes and observations' to log each feed time, sleep start and end, and nappy type as the day unfolds. At the foot, fill in the 'Three things to remember' box with the next feed time, the day's nappy count, and one settling method that worked so you can try it again.

What is printed on the page

Tips

Use a consistent shorthand — 'BF' for breastfeed, 'FF' for formula, 'W' for wet nappy, 'S' for soiled — to write quickly at 3 a.m.

Date every page and file them in order; a week's worth shows patterns in sleep clusters that are hard to see day by day.
