What this sheet is for

This sheet is for anyone providing hands-on care at home — a family member, a private aide, or a rotating team sharing a single care recipient's day. Reach for it first thing in the morning to map out the full day: who arrives when, what personal care tasks fall at what hour, which vitals need checking, and what the evening shift needs to know before you hand over.

How to use it

Start by filling in 'Today's top 3 care goals': one for any appointment or procedure, one for a symptom or condition to monitor, and one urgent supply or errand. Work down the 'Care schedule' column in 30-minute blocks from 6 am to 9 pm, writing each task, visit, or routine (morning wash, meal, therapy, rest period) at its planned time. Tick off items in the 'Daily care checklist' as the day progresses. At the end of the day, use 'Observations and handover notes' to record anything the next carer — or tomorrow's version of you — must know.

What is printed on the page

“Home Care Day Planner” is laid out as time-blocked day: hourly schedule column 6am–9pm on the left; top-three priorities, a to-do checklist and a notes box on the right. Under the title it reads “Hour-by-hour schedule, priorities, and care notes for one day of in-home caregiving.”, and the line at the foot of the page says “Before bed, note any changes in condition, mood, or appetite — small shifts over days reveal patterns.”.

The priority prompts slot holds three entries — “Appointment / procedure today”, “Symptom or concern to watch” and “Supply or task that cannot wait” — so the sheet is already filled in for caregiving rather than left as blank rows to label yourself.

The remaining labels on “Home Care Day Planner”, the caregiving sheet, are “Care date” for the date label, “Care schedule” for the schedule heading, “Today's top 3 care goals” for the priorities heading, “Daily care checklist” for the todo heading and “Observations & handover notes” for the notes heading.

Two things about this particular time-blocked day are settings rather than text, and “Home Care Day Planner” has them set like this. Half hours is on (the opposite of the layout's default): split each hour into two 30-minute rows. Notes box is on: include the notes box under the checklist.

Tips

Write the care recipient's meal and fluid times in the schedule first, then fit care tasks around them.

Use the checklist for recurring daily tasks — personal hygiene, repositioning, skin checks — so nothing is skipped during a busy shift.

Date and keep completed sheets in a folder; a week of pages shows patterns in sleep, appetite, or pain levels faster than memory alone.
