What this sheet is for

A personal best moment — a heavier lift, a faster run split, more push-ups in a set — deserves something more permanent than a note in a training log. This certificate is for coaches, personal trainers, gym partners, or self-directed athletes who want to mark the exact date a measurable record fell, whether in a one-rep max, a timed distance, or a bodyweight test.

How to use it

Write the athlete's name on the name line beneath 'This certifies that', then fill in the achievement line with the specific test — for example, '5 km run' or 'deadlift one-rep max' — and use the detail line to note the new result and the margin by which the old record was beaten. Record the exact date in the 'Date achieved' field. The Trainer or Coach signs on the left foot line; a training partner or witness signs on the right. Press a sticker or ink stamp into the circular seal to make it official.

What is printed on the page

“Personal Best Certificate” is laid out as modern band: a tinted band across the top carrying the title and presenter line, a heavy bar down the left edge, the name and achievement lines ranged left, and date and signature lines along the foot with the seal at the right.
