What this sheet is for

When care is shared between family members, agency workers, or day and night shifts, information gets lost in conversation. This sheet gives one carer a structured place to record what happened and what still needs doing, so the next person walks in knowing exactly where things stand — without a lengthy phone call or a written note that covers only half the picture.

How to use it

Start by writing today's date and shift in the Date / Shift field. Use Most important thing to communicate today to name the single fact the next carer must know — a new symptom, a missed meal, a scheduled call. Under Must happen today, write three tasks that cannot be handed forward: a pending GP call-back, a dressing change due at a set time, a hospital transport. Use Do if time allows for lower-priority errands. Work through the five Daily care checks during your shift and tick each one. Use Handoff notes for anything that does not fit the other sections — a change in behaviour, a supply running low, or a question for the doctor.

What is printed on the page
